Astera’s Cancer Survivorship Program is designed to help cancer survivors shift from disease-focused treatment to wellness-focused care.
It is important to meet with your medical oncology provider to develop a customized follow-up care plan to help you:
- Manage physical and emotional changes you may experience after your cancer treatment.
- Help you process your cancer journey.
- Adjust to your new normal post-treatment.
Your Initial Survivorship Visit May Include:
- Reviewing your cancer diagnosis and treatment journey to-date
- Establishing expectations re: your cancer diagnosis, treatments, and side effects (long-term)
- Completing assessments to identify your physical, emotional, cognitive, psychological, and social needs
- Creating management strategies to help you cope with physical, emotional, and social effects of your cancer treatment
- Assessing your risk of cancer recurrence or other types of cancers, based on the latest surveillance guidelines
- Providing referrals to cancer screening services and follow-up care, if needed
- Recommending strategies to improve your health through wellness (including nutrition, movement, and smoking cessation)
- Providing referrals for experts to address any barriers to wellness, such as dietitians, mental health providers, physical therapists, and integrative medicine specialists
