How to Register for Patient Health Navigator

To register for Personal Health Navigator, please confirm with our staff that we have your correct email address on file. Once confirmed, you’ll receive an automatic invitation email with instructions to complete registration.

After signing up, you can access your account anytime by visiting:  https://cjoc.phn.care.

Personal Health Navigator is accessible anytime, anywhere - from your computer, tablet, or mobile device. Patients can enjoy 24/7 access to their clinical and personal health information. The portal connects directly to your medical records at our practice, allowing you to:

  • Review and confirm appointments.
  • Send secure messages to your care team.
  • View clinical documents and access lab, pathology, and radiology reports.
  • Complete electronic patient forms.
  • Attend scheduled telehealth visits directly through the portal.
  • Access additional features to help manage your care.
